I did this deal just a little bit ago. The rebate reads "Limit one refund per name and address"
My rebate says one per household and they only refund you what you actually paid for the item up to 3.99 minus sales tax. The thinner headband is 3.99 at WAGS so it seems like it would be better to save your coupon for another purchase.

Got two of these in two transactions yesterday (7/3) at our local Wags. Each time the cat printed the rebate form.
i bought the 4.99 one and i didn't get a rebate form either.
